Designing A Service

If the admissions panel felt that the person can be offered a service we will contact the referring authority to confirm that we will complete an Individual Service Proposal.

The Individual Service Proposal will be completed by Modus and then shared with relevant parties e.g. the person being referred, the referring authority, family etc for discussion/ amendment if necessary.

Once the Individual Service Proposal has been agreed, Modus will attach costs to the package and contact the referring authority with these.

On the assumption that costs, service design and timescales are agreed by Modus and the referring authority, a place will then be offered.
